What is the procedure to claim Prize amount ?
The prize winner of a lottery shall surrender the prize winning ticket within 30 days of the draw with all necessary documents.Prize winner upto Rs.5000/- will be paid by the District Lottery Officers concerned.Prize winning tickets above Rs.5000/- shall be surrendered to the Director of State Lotteries after affixing the Signature,name and address of the prize winner on the reverse of the ticket with the following documents
1. A claim application along with a Photostat copy of the signed ticket on both sides duly attested by a Gazetted Officer/Notary.
2.Two Passport size photographs of the prize winner duly attested by a Gazetted/Notary.
3.A receipt for the prize money in the prescribed Form (Form No:3) affixing a revenue stamp worth Re.1/- with full address of the prize winner.
4.If the prize winner is a minor,Guardianship Certificate from a competent authority.
5.In joint claims, one of the prize winners should be authorised to receive the prize winners should be authorised to receive the prize money and a 'Joint Declaration' in stamp paper worth Rs.50/- should be executed in this regard.
6.An attested photocopy of the PAN if available.
Prize amount can also be claimed through Nationalised,Scheduled or Co-operative banks.The Prize ticket should be surrendered to the bank with all documents as noted above.The bank should submit the claim to the Director of State Lotteries with the following certificates.
a) The Letter of Authorisation from the Prize winner (Annexure-3)
b)The certificate from the receiving Bank.(Annexure 4)
c)The certificate from the collecting bank.(Annexure-5)
Income Tax and surcharge on prize applicable from time to time shall be deducted and Credited into the central Government account.
Prize winners who won car as prize,should submit a receipt for the car and a declaration to wait till the date of delivery of the car to remit the excess amount,if any hike in price occur after the date of booking.
